Foucault argues that the problem of simulation permeates the entire history of modern psychiatry. Ingenieros participates in the epistemological and political struggle that took place between experts-psychiatrists and simulators over the question of truth. The problem of the simulation of madness seems to be contemporaneous with the birth of modern psychiatry. We see this problem appearing in the principal texts of the history of psychiatry throughout the nineteenth and early twentieth centuries. Later, in the mid-nineteenth century, it was a topic of concern to degenerationists. They both published texts and studies referring to actual cases of simulation that aimed to provide strategies for unmasking simulators — subjects who raised permanent questions about the possibilities, reach and limits of psychiatric power.

His theories addressed the relationship between power and knowledge, and how they are used as a form of social control through societal institutions. Though often cited as a post-structuralist and postmodernist, Foucault ultimately rejected these labels, preferring to classify his thought as a critical history of modernity.
